2008 All-State Nomination Form

Instructions to be considered for an All-State selection, this form must be completed
in full for each player nominated.  Four players may be nominated from one team but
only three may be chosen for the All-State team.  Include a senior picture (preferably
black and white) and return the form to your board of directors representative no
later than Thursday, May 8th, 2008.  Coaches must be a member of the Slow Pitch Assoc.
